Found in the Elderly & Disabled Waiver Program, the Community Care Services Program (CCSP) and Service Options Using Resources in a Community Environment (SOURCE) , and serve frail elderly and disabled Georgians otherwise eligible under a nursing facility level of care through the provision of case management for service coordination, adult daycare, alternative living services, personal care, home-delivered meals, and respite care for family caregivers. Enhanced case management service is available to individuals who require Veterans Administration – Home and Community Based Services - The Veteran-Directed Home and Community Based Services (VD-HCBS) program is the result of a partnership between the Department of Health and Human Services/Administration for Community Living (ACL) and the VHA. The program provides Veterans of all ages the opportunity to receive participant-directed home and community based services, enabling them to avoid nursing home placement and to continue to live in their homes and communities. Services Provided: Personal Support Services Medication Management Residential Services Family Model Personal Care Homes Learn how to Become a Licensed Personal Care Home Provider in Georgia Group Model Personal Care Homes Developmental Disability Services Home Care & Residential Services Intellectual Disability Waiver (NOW and COMP) The New Options Waiver (NOW) and Comprehensive Supports Waiver Program (COMP) offer home- and community-based services for people with a developmental or intellectual disability. A diagnosis of developmental disability includes intellectual disability or other closely-related conditions such as cerebral palsy, epilepsy, autism or neurological problems. These are developmental disabilities that require a level of care provided in an intermediate care facility for people who have intellectual disability. There are a little over 12,000 people with developmental disabilities served by the NOW/COMP programs in Georgia. Services Provided: Community Living Supports Community Living Arrangements Host Homes Learn how to Become a Developmental Disabilities Host Home Provider Community Access Individual Community Access Group Skilled Nursing Services Behavioral Services where available Independent Care Waiver Program The Independent Care Waiver Program (ICWP) offers services that help a limited number of adult Medicaid members with physical disabilities live in their own homes or in the community instead of a hospital or nursing home. ICWP services are also available for persons with traumatic brain injuries (TBI). The program operates through the Georgia Department of Community Health (DCH) under a Home- and Community-Based Waiver (1915c) granted by the Centers for Medicare & Medicaid Services (CMS). Services Provided: Personal Support Services Alternative Living Services (Family Model Personal Care Homes) Skilled Nursing Services Money Follows the Person Money Follows the Person (MFP) is a rebalancing initiative that was made possible by an eleven-year grant to states from the Centers for Medicare and Medicaid Services (CMS). This grant is designed to help individuals who are institutionalized in in-patient facilities like nursing facilities, hospitals and intermediate care facilities for individuals with intellectual and developmental disabilities (ICF/IDD) return to their homes and communities. Services Provided: Grocery and Household Goods Shopping Personal Support Services Medication Management Georgia Pediatric Program (GAPP) Provides services to medically fragile children with multiple system diagnoses. Services are provided in their homes and communities and include nursing services to address the need for skilled professional services and personal care support services that assist with activities of daily living. The individuals being cared for cannot be related to the owner or administrator by blood or marriage. Personal Care services include, but are not limited to, individual assistance with or supervision of self-administered medication(s), assistance with ambulation and transfer, and essential activities of daily living such as eating, bathing, grooming, dressing and toileting.
